REQUEST FOR
LETTERS OF INTEREST
The Office of the Clermont County Engineer is seeking professional services from interested engineering firms for the following public improvement project.
Eastgate South Drive and Clepper Lane Roundabout
The intersection of Eastgate South and Clepper Lane is located in Union Township. This intersection area accommodates traffic from three public approaches and two private property access drives. The project will extend approximately 500 feet in each direction from the center of the intersection, with additional work being done southward on Clepper Lane to create a landscaped median and center turn-lane.
This project will remove the existing signalized intersection and replace it with a roundabout. The project will match the existing 12′ lane widths. Curb, catch basins, and additional culverts will tie into the existing storm drainage system. Two private drives and three public road connections are anticipated to be accommodated by the roundabout in order to enhance safety and reduce congestion. See Conceptual Plan posted to the Clermont County Engineer’s website, clermontcountyengineer.org.
The successful professional design firm(s) shall provide such services in accordance with Section 153.65 of the Ohio Revised Code.
Interested firms shall provide a Letter of Interest (3 copies). LOI shall be limited to ten (10) 8 1/2″ x 11″ single sided pages plus two (2) pages per project for the combined Key Staff and Project Approach, unless otherwise noted in the Project Description. All pages including the transmittal letter count against the limit. The submittal must include valid copies of the Workers’ Compensation Certificate and Certificates of Insurance.
The LOI content shall at a minimum include:
A brief history of the firm which shall include information pertaining to major areas of expertise and applicable ODOT prequalification’s.
A list of representative projects and clients including project costs for contracted projects within the past five (5) years.
All surveying/design to be completed in State Plane, Ohio South, NAD83, NAVD88 with map units of U.S. Survey Feet.
Provide Project Approach, Project Manager and other key staff members, including key sub-consultants.
This information must be filed with the Office of the Clermont County Engineer, Attention: Craig Stephenson, 2381 Clermont Center Drive, Batavia, Ohio 45103 by no later than 4:00 p.m. on Friday, October 5th, 2012. Mr. Crum moved for adoption of the following resolution:
RESOLUTION ESTABLISHING REVISED FEES FOR ADMINISTRATION OF VACCINES TO CHILDREN AND FLU SHOTS
WHEREAS, the Board of Health of the Clermont County General Health District has the authority pursuant to Ohio Revised Code Section 3709.09 and 3709.21 to establish a uniform system of fee services; and
WHEREAS, the Board of Health of the Clermont County General Health District is mandated to provide services that protect the public health and prevent or restrict disease, and abate or suppress nuisances; and
WHEREAS, the Board of Health of the Clermont County Health District has responsibility to insure that services are provided for the prevention of vaccine preventable disease; and
WHEREAS, the Board of Health of the Clermont County General Health District has determined that it is necessary to increase the fees for administration of vaccines administered to child and to increase the fee for flu shot; and
WHEREAS, the Board of Health of the Clermont County General Health District realizes that some Clermont County residents who are eligible for free vaccine through the Vaccine for Children program may not have the ability to pay for the administration fee.
NOW, THEREFORE, BE IT RESOLVED, by the Board of Health of the Clermont County General Health District, that fees for the following services be revised as stated:
Vaccines to children – Cost of vaccine plus $10 administration fee per injection
Flu Shots – $20
NOW, THEREFORE, BE IT FURTHER RESOLVED, that all or part of the administrative fee for individuals receiving free children’s vaccine through the Vaccine for Children program may be waived when the individual is unable to pay for the entire administrative fee.
NOW, THEREFORE, BE IT FURTHER RESOLVED, that all fees revised pursuant to this Resolution hereby become effective on October 1, 2012.
NOW, THEREFORE, BE IT FURTHER RESOLVED, by the Board of Health of the Clermont County General Health District, that the revised fees be recorded and certified to become a part of the record of local regulation by the Board of Health of the Clermont County General Health District.
NOW, THEREFORE, BE IT FURTHER RESOLVED, by this Board of Health of the Clermont County General Health District, that this regulation be published in one newspaper of general circulation that is in the Clermont County General Health District once a week for two consecutive weeks and that the first publication take place no later than 10 days prior to the effective date of the revision adopted herein.
NOW, THEREFORE, BE IT FURTHER RESOLVED, that this Board of Health of the Clermont County General Health District find and determine that all formal actions relative to the passage of this resolution were taken in an open meeting of this Board, and that all deliberations of this Board and of its committees, if any, which resulted in formal action, were taken in meetings open to the public, in full compliance of the applicable legal requirements, including section 121.22 of the Ohio Revised Code.

The Unknown Heirs, Devisees, Legatees, Executors, Administrators, Spouses and Assigns and the Unknown Guardians of minor and/or Incompetent Heirs of Alma Jean Dobbs, whose last known address is unknown, and cannot by reasonable diligence be ascertained, will take notice that on the 3rd day of July, 2012, Defendant, OneWest Bank FSB filed its Answer and Cross-Claim in the Common Pleas Court of Clermont County, Ohio in Case No. 2012 CVE 01064, on the docket of the Court, and the object and demand for relief of which pleading is to foreclose the lien of plaintiff’s mortgage recorded upon the following described real estate to wit:
Property Address: 5984 Meadow Creek Drive Apartment 8, Milford, OH 45150 and being more particularly described in plaintiff’s mortgage recorded in Mortgage Book 1550, page 794, of this County Recorder’s Office.
All of the above named defendants are required to answer within twenty-eight (28) days after last publication, which shall be published once a week for three consecutive weeks, or they might be denied a hearing in this case.
